All refill requests are individually reviewed by your medical provider and are subject to provider discretion, clinical appropriateness, and applicable prescribing regulations. Refill requests are available for current Renaissance Longevity members continuing an existing provider-directed treatment plan. Submission of this form does not guarantee that a refill will be approved.
